This template routes every entry through one form, so each team lands in your responses complete: contact details, roster, division, waiver status, and entry fee, all in a single pass that takes captains about 8 to 12 minutes.
Seeding a bracket from complete entries instead of email threads
Picture a weekend volleyball tournament with 24 teams across skill levels. Instead of sorting entries by hand, each captain picks a division on the form, from Recreational up to Elite / Professional, and an age category that runs from Youth (Under 14) to Open (All Ages). Two required radio questions do quiet but important work: "Has your team participated before?" flags first-time teams that may need extra communication, and "Do all team members have signed waivers?" forces a straight answer, either "Yes, all waivers are signed" or "Not yet, will submit before tournament", which gives you a follow-up list instead of a surprise on game day.
The optional fields stay out of the way. Coach or manager name, a team photo upload, and a box for special requests or accommodations are there for teams that need them and skippable for teams that don't.
Adjust the divisions, keep the structure
