Tournament entries rarely arrive in one piece. A captain emails a roster, someone texts a headcount, and three days before the bracket is drawn you're still chasing waiver confirmations across four inboxes. This template routes every entry through one form, so each team lands in your responses complete: contact details, roster, division, waiver status, and entry fee, all in a single pass that takes captains about 8 to 12 minutes.

Seeding a bracket from complete entries instead of email threads

Picture a weekend volleyball tournament with 24 teams across skill levels. Instead of sorting entries by hand, each captain picks a division on the form, from Recreational up to Elite / Professional, and an age category that runs from Youth (Under 14) to Open (All Ages). When registration closes, you filter responses by those two answers and your pools practically build themselves. The form's structure keeps captains moving: a cover page sets expectations, two content pages gather the details, a payment page handles the entry fee, and an ending page confirms the submission.

The roster, waiver, and captain details every entry must include

The template ships with 13 fields, and the required ones cover exactly what a tournament director needs before seeding anyone. Captains give their team name, their own name, email, and phone, plus a headcount and a full roster in a "Team Roster (list all player names)" box. Two required radio questions do quiet but important work: "Has your team participated before?" flags first-time teams that may need extra communication, and "Do all team members have signed waivers?" forces a straight answer, either "Yes, all waivers are signed" or "Not yet, will submit before tournament", which gives you a follow-up list instead of a surprise on game day.

The optional fields stay out of the way. Coach or manager name, a team photo upload, and a box for special requests or accommodations are there for teams that need them and skippable for teams that don't.

Adjust the divisions, keep the structure

Every field is editable, so a golf scramble can swap the roster box for foursome slots and an esports league can rename divisions to match its ranked tiers. You can adjust the age categories, reword the waiver question to cite your own liability form, and put your event's colors and logo on the whole thing.
