Registration night, a clipboard, and one unreadable allergy note

Fourteen fields and roughly 10 to 15 minutes of a parent's evening: that is everything this template needs to turn an interested family into a confirmed camper. Compare that with the usual routine. A PDF goes out by email, half the copies come back photographed at an angle, and a week before opening day someone is still phoning families to ask whether the allergy note says peanuts or shellfish. This form moves the whole exchange online, so every answer arrives typed, complete, and attached to the right child.

The template's own description states the job plainly: simplify camp enrollment with health forms, activity preferences, and emergency contacts. The 14 fields deliver on all three.

Guardian details, session picks, and the swim question

Identity comes first in importance: parent or guardian name, email, and phone, then the camper's full name and date of birth, all required. A Camp Session dropdown lets families commit to Session 1 in June, Session 2 in July, Session 3 in August, or the full summer. An Activity Preferences checklist covers Swimming, Arts and Crafts, Hiking / Nature, Sports, Music and Drama, and STEM Activities, so grouping decisions can start before anyone steps off the bus.

The safety block is where paper forms usually fail, and it gets the most care here. There is an open text area for allergies or medical conditions, a dietary requirements dropdown with options running from vegetarian to nut allergy, a required emergency contact name and phone, and a field naming the authorized pickup person. A required three-option question, "Can the camper swim?", separates strong swimmers from basic and non-swimmers, and an optional closing text area catches anything else a parent wants staff to know.

Structurally, families move through a cover page, three content pages, a payment page, and an ending page. At this length the traditional page-by-page mode is the recommended fit, and you can keep the progress bar on so parents always see how much is left.

From submissions to bunk lists, kitchen counts, and lifeguard ratios

Every registration lands in a response dashboard with per-question breakdowns, so you can read session demand at a glance: if Session 2 is filling twice as fast as Session 3, you will know before it becomes a waitlist problem. The dietary dropdown becomes the kitchen's headcount, the swim question becomes the waterfront staffing plan, and the activity checklist tells you whether you need a second arts tent this year.

When it is time to build rosters, export everything to CSV and sort by session, age, or activity. If your camp runs on other software, webhooks and a Zapier connection can push each new registration out the moment a parent hits submit.

Putting the form where camp families already are

Publish the form and share it as a direct link in your spring email to returning families, print the QR code on flyers for schools and community boards, or embed it on your camp website with the snippet. Sites on WordPress can drop it in through the Uplup plugin's shortcode. Before any of that, swap in your camp's logo and colors so the first thing parents see is yours, and rename any field, option, or session to match this year's program.
