Registrants get through the form in 3 to 5 minutes, and you get far more than a name on a list. You learn each person's skill level, whether they meet your prerequisites, which session they want, and what they actually hope to learn.
One workshop, three skill levels, no surprises on the day
Picture a hands-on session, pottery, Python, sourdough, whatever you teach. The subject barely matters because the failure mode is always the same: half the room has done it before and drifts, the other half has never touched the material and drowns. This template is built to prevent that morning. Before anyone walks in, every registrant has identified as Beginner, Intermediate, or Advanced, answered a direct prerequisite question, and committed to the morning session, the afternoon session, or the full day.
From contact details to dietary needs, in one pass
The basics come first. Full name and email are required, phone number is optional, and a company or organization field tells you when an employer is sending a group. The required Skill Level dropdown does the sorting, and the Workshop Session field offers three concrete choices: morning from 9am to 12pm, afternoon from 1pm to 4pm, or the full day.
The prerequisite check asks it straight: "Do you have prerequisite experience?" Registrants answer "Yes, I meet all prerequisites," "Partially," or "No, I am completely new." A materials checkbox lets each person request printed handouts, digital PDFs, access to recordings, or any combination. Dietary requirements get a dropdown of their own, and two open text boxes close things out: one asks what they hope to learn from the workshop, the other asks about special accommodations. The page flow runs from a cover page through content and a payment step to an ending page, so paid workshops can collect the fee inside the registration itself.
Reading the roster before you plan a single exercise
Every submission lands in your Uplup dashboard, where per-question breakdowns turn the roster into a planning document. If most of the room answered "Partially" on prerequisites, you build in a refresher block. If nobody ticked printed handouts, you cancel the print run. If the free-text answers about learning goals cluster around one topic, you weight the agenda toward it.
When you need the data elsewhere, export the full response list to CSV for check-in day, or push each new registration to your other tools as it arrives through webhooks or Zapier.
A registration link that travels as far as your flyer does
Share the form as a direct link in your announcement email, drop a QR code on the studio poster, or post it straight to social. If you run a website, paste the embed snippet into any page, and WordPress sites can use the shortcode from the Uplup plugin instead. The template's recommended conversational mode shows one question at a time, which suits a short form filled out on a phone between meetings.
