It's built for credentialing programs that need more than a name and an email before they can seat someone for an exam, handling eligibility screening, scheduling preferences, and credential verification in a single pass.

The structure runs cover page, content page, payment page, ending page, so candidates confirm their details and settle the exam fee in the same sitting. Traditional mode is the recommended setup here, keeping the full question list visible instead of presenting one item at a time, which suits candidates who want to review everything before they pay.

From legal name to ID document in one pass

Identity comes first: a required Full Name field labeled "as it appears on ID", plus email and phone. That labeling matters, because a mismatch between the registration record and the ID a proctor checks is one of the most common reasons candidates get turned away on test day.

From there, a Certification Exam dropdown offers four tracks, from Level 1 Foundation through Level 3 Expert plus a Renewal / Recertification option. A Preferred Testing Location select covers online proctored testing alongside centers in New York, Chicago, and Los Angeles. Two required radio questions screen for previous certifications and testing accommodations, with preset choices for extended time or a separate room. An optional field captures the current certification number for renewing candidates, a required ID document upload verifies identity, and an open notes box catches anything else.

Sorting candidates into rooms, tracks, and accommodations

Every submission lands in your response dashboard, where per-question breakdowns show demand at a glance: how many candidates picked each exam level, which testing locations are filling fastest, and how many accommodation requests you need to arrange before test day. Export the full set to CSV to build your roster, or push each registration into your scheduling or credentialing system as it arrives using webhooks or Zapier.

Swap in your own exam levels, cities, and branding

The four certification tracks and three testing cities are placeholders for your program's real structure, and every option is editable, so you can rename levels, add centers, or trim the accommodations list to what you actually offer. Add your organization's logo and colors so the form matches the rest of your candidate communications, then share it by link or QR code, embed it on your site, or drop it into a WordPress page with the Uplup plugin's shortcode.
