You want more than a name on a list: a complete profile, a billing choice you can act on, and a record of how they found you. This template gives both sides what they came for in an estimated 5 to 8 minutes, and it hands you the details that turn a signup into a paying, renewing member.

Turn each signup into a billing-ready member record

Every submission lands with the big decisions already made. The Membership Plan field asks joiners to choose between Basic, Standard, Premium, and Family tiers, and the Billing Frequency question settles monthly, quarterly, or annual payment, with the annual option flagged as a 15 percent saving. No follow-up emails asking which plan they meant or how often to charge them.

From there, response analytics break results down per question, so you can see which tier new members choose most and how many said yes to your newsletter. Export submissions to CSV for your membership database, or send each one to the tools you already run through webhooks and Zapier.

Twelve fields that add up to a complete member profile

The form runs four pages, a cover page, a content page, a payment page, and an ending page, with 12 fields in total. Identity comes first: full name, email, phone, date of birth, and mailing address are all required, so every record in your roster is complete enough to bill and mail.

The rest maps out who this member is and why they joined. An Areas of Interest checkbox offers options like Events & Meetups, Networking, and Discounts & Perks, so you know which benefits to point new members toward. A 'How did you hear about us?' question tracks where joiners come from, an optional referral code field credits the person who sent them, and a required newsletter choice keeps your mailing list honest. An open comment box at the end catches anything the other fields missed.

Reshape the tiers, the prices, and the whole flow

Your organization does not charge $9.99 a month for a Standard plan just because the template does. Every field, option, and price label is editable: rename the tiers, adjust the amounts, add a student rate, or drop the phone requirement if you never call anyone.

The template recommends conversational mode, which presents one question at a time and suits a form asking for personal and billing details. Turn on the progress bar so members can see how close they are to done, allow back navigation for second thoughts on plan choice, and apply your own colors and logo so the form reads as yours.
