This club membership application pulls the whole intake into one link: personal details, membership category, a referral check, and dues payment, all collected in a single pass that takes applicants about 8 to 10 minutes.
Ten fields, from date of birth to referral name
The application collects the basics first: full name, email, phone, and date of birth, all required. An optional occupation field adds context without forcing it. Applicants then pick a membership type from four options, Individual, Family, Corporate, or Student, so dues and privileges are matched from the start.
The screening happens in the second half. A "Referred By" field records which current member vouched for the applicant. A required interests checklist covering social events, sports and fitness, dining, business networking, and community service shows where a new member would plug in. Finally, a required long-form question, "Why would you like to join?", gives your committee something real to read, with an optional field for anything else the applicant wants to add.
Structurally, the template runs as a cover page, two content pages, a payment page for dues, and an ending page. It ships in a traditional page-by-page layout rather than conversational mode, which suits a formal application better than a chat-style flow.
