Alumni Network Registration

"How would you like to participate?" is the question doing the real work in this template. Graduates tick any mix of five options: mentoring students, attending events, guest speaking, donating or fundraising, and networking only. That one checkbox tells your alumni office who wants a stage, who wants a nametag, and who just wants the newsletter.

From graduation year to a message for fellow alumni

The form holds 11 fields across a cover page, two content pages, and an ending page. Only four are required: full name, email, graduation year, and degree or program. That keeps the barrier low for a graduate filling this out on a phone between meetings.

The optional fields are where the network gets useful. Current employer, job title, city, and a LinkedIn profile URL turn a mailing list into a searchable career map. A free-text box invites a brief bio or message to fellow alumni, which hands you ready-made material for a class notes column. Uplup estimates 5 to 7 minutes to complete, and the template is set up for conversational mode, one question per screen, which reads more like catching up than filling in paperwork.

The offices that chase down graduates

University advancement teams are the obvious fit, but the same structure works for high school reunion committees tracking down the class of 1998, coding bootcamps building a hiring pipeline from past cohorts, and professional associations that want members' employers and titles kept current. Anywhere the question is "where did our people end up, and who will come back to help," this form answers it.

Rename the degree field, keep the structure

Every field label, requirement, and checkbox option is editable, so "Degree / Program" can become "Cohort" for a bootcamp or "House" for a boarding school. Add your institution's colors and logo, then publish wherever alumni already are: a direct link in your newsletter, a QR code on reunion invitations, an embed on your alumni page, or a WordPress shortcode through the Uplup plugin.

Responses land in a dashboard with per-question breakdowns, so you can see at a glance how many graduates volunteered for mentoring versus guest speaking. Export the whole list to CSV for your CRM, or push each new registration to other tools through webhooks or Zapier. Publishing requires a Pro plan, and once the form is live you can keep editing it without breaking the link.
