In conversational mode, each field becomes its own page for a focused experience.

Store owners chasing social proof know the problem: customers love the product, but nobody writes it down. This template exists for exactly those sellers, the Shopify shop with ten SKUs and three reviews, the DTC brand that wants quotable praise for its landing pages, the small marketplace that needs ratings it can compare across products. It collects a complete, publishable review in one pass, and completion runs 3 to 5 minutes.

Built for shops that turn feedback into sales copy

Ecommerce teams are the core users. They send it a week after delivery, when the product has been unboxed and used, and route the answers into product pages, ads, and comparison charts. Product managers read the same data differently: because Quality and Value for Money are scored separately, a product that earns 5 stars on quality but 2 on value tells you the price is the problem, not the product.

Agencies and marketplace operators pick it up too. The final question, "Would you recommend this product?", is a plain yes or no, which makes it easy to compute a recommend rate per product without any survey math.

Nine required fields, three of them star scales

The form opens with a name and email, so every review is tied to a reachable customer. A "Product Purchased" field connects each review to a specific item, which matters once your catalog grows past a handful of SKUs. Then come the three ratings: Overall Rating, Quality, and Value for Money, each on a 5-point star scale.

The written part carries the weight. A "Review Title" gives you the headline you'd display above the stars, and "Your Review" is an open textarea for the detail. Every one of the nine fields is required, so you never receive a bare star rating with nothing worth publishing alongside it.

The layout is two content pages plus an ending page, and the template's recommended conversational mode shows one question at a time, which suits customers filling it out on a phone. Every field, label, and rating scale stays editable if your catalog needs different wording.

Getting reviews without begging for them

The template is on Uplup's free plan, so there's no cost barrier to trying it on your next batch of orders. Share it as a direct link in your post-purchase emails, print a QR code on the packing slip so the ask arrives with the product, or embed the form on a "review us" page of your store. Stores built on WordPress get a shortcode through the Uplup plugin that places the form inside any page or post.

Once responses arrive, per-question breakdowns show your average scores on each of the three rating scales, and CSV export moves the raw reviews into whatever tool builds your product pages. Webhooks and Zapier can push each new review to your team the moment it lands.

Who Is This Template For?

This template works for a wide range of goals and industries.

Post-purchase review collection

Send the form link a week after delivery and gather a name, verified email, three star ratings, and a written review from every customer, tied to the exact product they bought.

Testimonial sourcing for product pages

The required Review Title and Your Review fields hand you a headline and body copy ready to display, so strong responses go straight onto landing pages and ads.

Product line diagnostics

Separate Quality and Value for Money scores plus the yes or no recommend question show which products underperform on price versus build, and CSV export makes the comparison across SKUs easy.

Key Features

Three separate 5-star scales

Overall Rating, Quality, and Value for Money are scored independently, so you can tell whether a low review is about the product itself or the price tag.

Conversational mode for phone reviewers

The template's recommended mode shows one question at a time across its two content pages, which keeps mobile customers moving through all nine fields.

Reviews you can actually use

Per-question breakdowns average each rating scale, CSV export moves full reviews into your publishing workflow, and webhooks or Zapier push new submissions to your team instantly.

Frequently Asked Questions

Is the product review form free to use?
Yes. It runs on Uplup's free plan, so you can customize it, share it with customers, and collect reviews without paying anything.
Can I change the three rating categories?
Yes. Overall Rating, Quality, and Value for Money are just the defaults. Rename them, adjust the 5-star scale, or swap them for criteria that fit your products, and edit any of the other fields the same way.
How long does it take a customer to leave a review?
About 3 to 5 minutes. There are nine fields in total, three of them quick star ratings, and conversational mode presents them one at a time so the form feels shorter than it is.
