The structure is two content pages followed by an ending page, so nobody faces a wall of questions all at once.
Scores at the front, reasons close behind
The form starts by identifying who is talking: a required name, an email address, and a Product/Service dropdown so feedback lands against the right item. That dropdown ships with Product A, B, C, and Other as placeholders. From there, the four rating scales grade the product on matching 5-point scales, and a radio question asks directly whether the respondent would recommend the product, with answers running from Definitely to Probably not.
Two optional fields handle the qualitative work. A checkbox question asks which features people use most, and a textarea asks what improvements they would suggest. Keeping these optional is deliberate: required essays hurt completion, but the users who do write usually have something worth reading. The closing NPS question, how likely are you to recommend us to a friend or colleague, gives you a benchmark number to track quarter over quarter.
Reading responses like a roadmap, not a report card

A high Ease of Use score sitting next to a low Value for Money score points at pricing, not usability. Cross-reference the most-used-features checkbox with the improvement suggestions and you can see whether complaints cluster around the features people actually rely on, which is where fixes pay off first.

Swap the placeholders for your actual product line
Every field is editable. Rename the dropdown options to your real products, replace Feature 1 through Feature 5 with the ones you ship, and adjust the rating labels if your team uses different language. You can also apply your own colors and logo so the form looks like it came from you, not from a form builder.
The template runs in traditional mode by default, which suits a form this compact, though you can switch to conversational delivery that shows one question at a time. Back navigation and the progress bar are optional toggles.
