Form Structure

Thank You Page

Multi-page layout keeps your form organized and easy to complete.

Local print shops, campus copy centers, and sign makers take orders from every direction: over the counter, by phone, in email threads with the attachment missing. This template was built for exactly that mess. It gathers a complete, chargeable print order in one pass, so nothing goes to press on a guess.

Who runs their order intake on this

The obvious fit is a small commercial printer or a franchise print counter that wants a standard intake instead of a notepad by the register. It also works for in-house teams: a marketing department that produces flyers, posters, and banners for the rest of the company can publish it internally and stop fielding requests through chat. Event producers who order signage from a preferred vendor use it in the other direction, sending the same structured brief with every job.

Ten answers, one press-ready job ticket

The form runs two content pages, then a payment page, then an ending page, and holds 10 fields in total. Contact basics come first: name and email are required, phone is optional. The order itself lives in four required fields. Print Type offers six options, from Business Cards and Flyers through Posters, Brochures, and Banners, with Other as a catch-all. Quantity is a plain number field, Paper Size covers Standard, Letter, Tabloid, and Custom, and Color lets the customer choose Full Color, Black & White, or Spot Color.

Three optional fields round out the ticket. A Needed By date flags the deadline, an Upload Design File field lets customers hand over artwork, which attaches to the response as a link, and a Special Instructions box catches the bleed, folding, and finish notes that would otherwise arrive as a second phone call. Uplup estimates 5 to 8 minutes to complete. Every field is editable, so a shop that stocks specific papers can rewrite the Paper Size options to match what is actually on the shelf. Publishing requires Uplup's Pro plan, which is what covers the payment step.

Orders from the counter, the website, and the inbox

Print a QR code for the register and the storefront window so walk-ins can start their order from a phone while they wait. Embed the form on your website with the snippet, drop the WordPress shortcode in if your site runs the Uplup plugin, or send the direct link any time a customer emails to ask how ordering works.

On the back end, every submission lands in response analytics with per-question breakdowns, so you can see which print types dominate and where deadlines cluster. Export orders to CSV for the job board, or push each new request into your production tracker through webhooks or Zapier. Add your logo and brand colors before publishing so the form reads as your shop, not as a template.

Who Is This Template For?

This template works for a wide range of goals and industries.

Print shop counter intake

Replace the notepad by the register with a structured order that captures print type, quantity, paper size, color, and deadline before the job is quoted.

Internal marketing print requests

In-house marketing teams publish the form so other departments submit flyer, poster, and brochure requests as complete briefs instead of chat messages.

Event signage and banner orders

Event producers send vendors the same structured banner order every time, with the artwork attached as a link and rush dates flagged in the Needed By field.

Key Features

Six print types built in

The required Print Type field ships with Business Cards, Flyers, Posters, Brochures, and Banners, plus an Other option, and you can edit the list to match your equipment.

Payment page in the flow

After the two content pages, customers reach a payment page, so the order and the charge happen in the same pass instead of a follow-up invoice.

Deadline and artwork up front

An optional Needed By date, a design file field that attaches artwork as a link, and a Special Instructions box catch the details that usually surface too late.

Frequently Asked Questions

Can customers send their artwork with the order?
Yes. The form includes an optional Upload Design File field, and the artwork attaches to the submission as a link, so you open it straight from the response view. Customers who don't have final files yet can skip it and describe the job in Special Instructions instead.
Can I change the print types and paper sizes to fit my shop?
Yes. The Print Type list starts with Business Cards, Flyers, Posters, Brochures, Banners, and Other, and you can rewrite, remove, or add options in the editor. The same goes for Paper Size, which ships with Standard, Letter, Tabloid, and Custom, and for the three Color choices.
Do customers have to set a deadline?
No. Needed By is an optional date field, as are phone, the design file, and special instructions. What the form does require is name, email, print type, quantity, paper size, and color, so every submission arrives quotable even when the deadline is flexible.
