Home bakers taking orders through Instagram need somewhere to send people besides their inbox. Storefront bakeries need a way to take custom orders without tying up staff on the phone. Event cake makers need enough detail up front to quote accurately before the first consultation. This template serves all three, and it asks a realistic amount of effort from customers: most finish in 6 to 10 minutes, which is short enough to complete on a phone and long enough to capture a real design brief.
The order flows across a cover page, two content pages, a payment page, and an ending page, so customers get a proper storefront experience rather than a wall of inputs. It runs in traditional page-by-page mode by default, though you can switch to conversational mode if you prefer one question at a time.
Size, flavor, frosting: the ten answers that define a cake
The form's 10 fields start with the basics: full name and email are required, phone is optional, and a date picker captures the pickup or delivery date. Then come the decisions that actually shape the bake. Cake Size offers five choices from 6 inch up to Tiered. Flavor covers Vanilla, Chocolate, Red Velvet, Lemon, and Carrot, plus a Custom option for requests off the menu. Frosting lets customers pick between Buttercream, Fondant, Cream Cheese, and Whipped Cream.
The last three fields are where guesswork goes to die. Message on Cake captures the exact inscription, spelled the way the customer wants it. Design Description gives them room to describe colors, themes, and styles in their own words. And the Design Reference Photo field lets them point you to inspiration; the reference comes through attached to their response as a link you can open while you plan the build.
Your menu, your rules
None of the options above are fixed. If you don't work with fondant, remove it. If your signature flavor is pistachio rose, add it. Every field label, dropdown option, and required setting is editable, so the form can mirror your actual menu instead of a generic one.
