Every phone call that rings out during dinner service is a table that might sit empty. Your host can't take bookings while seating guests, and voicemail reservations get transcribed wrong, half-heard, or forgotten by the time the rush ends. This template moves the whole exchange online, so guests book their own tables and your team starts each shift with clean, complete requests instead of a stack of sticky notes by the phone.

An anniversary dinner, booked while you were closing out the till

A couple planning their anniversary finds your booking link at 10 pm, long after anyone stops answering the phone. They pick a date, choose the 7:00 PM slot from your list of service times, enter a party of two, and select Outdoor under Seating Preference. In the Special Occasion box they mention the anniversary, and under Dietary Requirements they flag a shellfish allergy. The whole thing takes two to three minutes, and the next morning your host reads a complete, legible booking with an email address attached, ready to confirm.

Nine questions that read like a host taking a booking

The form asks for nine things and nothing more. Full name and email are required, so every reservation comes with a reachable contact, while phone stays optional for guests who prefer not to share it. A date picker and a Time dropdown with eight slots running from 11:00 AM through 9:00 PM pin down the when, a number field captures the guest count, and Seating Preference offers Indoor, Outdoor, Bar, or No Preference. Two optional text boxes, Special Occasion and Dietary Requirements, give the kitchen and floor staff the context that turns a routine table into a good review.

The structure is deliberately light: one content page of questions followed by an ending page that confirms the request was received. The template's recommended conversational mode goes further and presents one question at a time, which feels closer to chatting with a host than filling out paperwork.

Bend the slots and seating to fit your room

The preset times are a starting point, not a rule. Edit the dropdown to match your actual seatings, drop the lunch block if you only serve dinner, or tighten slots to 30-minute turns on weekends. Rename the seating options to match your space, patio, chef's counter, private room, and add your logo and colors so the form looks like your restaurant rather than a generic widget.

Getting it in front of hungry guests is the easy part: share the direct link, print a QR code for the window or the menu, embed it on your site, or add it to a WordPress page with the Uplup shortcode.
