You get enough detail to price the work, or to decide the job isn't worth the drive. The Landscaping Service Request template collects nine answers and takes a respondent 6 to 10 minutes, which is short enough that serious prospects finish it.

The form runs as a cover page, two content pages, and an ending page. Contact basics are required where it counts: full name, email, and address, with phone optional. From there it moves to the job itself. A Service Type dropdown covers Lawn Mowing, Garden Design, Tree Trimming, Full Landscaping, and Maintenance. Property Size runs from Small Yard through Commercial, and Frequency pins down whether the client wants a one-time cleanup or weekly, bi-weekly, or monthly service.

The last two fields do the heavy lifting on pricing. A required Project Description gives clients room to explain the slope, the drainage issue, or the hedge that got away from them. An optional Photos of Area field lets them attach pictures as links, so you can see the site before you name a number.

Reading requests before you load the trailer

If most requests pick Maintenance at a monthly frequency, you're building a recurring route, not chasing one-off cleanups, and you can price accordingly. Export responses to CSV when it's time to quote in bulk, or push each submission to your scheduling or invoicing tools through webhooks and Zapier so nothing sits unanswered overnight.

Share the form as a plain link, or generate a QR code and put it on truck decals, yard signs, and leave-behind cards. If you have a website, drop the embed snippet on your contact page; WordPress sites can use the shortcode from the Uplup plugin instead.

Nothing here is locked. Rename fields, swap the service options for what you actually offer, add your logo and colors, and turn on the progress bar so respondents can see how close they are to done. If a request comes in thin, the answers you do have still tell you whether it's worth a callback.
