"Do you own or rent?" is a single radio button on this application, and it prevents more failed adoptions than any other question on the form. A renter whose landlord bans dogs can adore an animal and still have to bring it back. Asking before the meet and greet costs nothing, which is why this template asks early and pairs the answer with eleven other fields that build a full picture of the home a pet would be walking into.
From housing type to household headcount, what gets screened
The 12 fields divide into identity, environment, and history. Contact details come first: full name, email, phone, and home address, all required. Then the environment questions take over. Applicants pick the type of pet they're interested in from Dog, Cat, Rabbit, Bird, or Other, choose a housing type ranging from House with Yard to Apartment / Condo to Farm / Rural, state whether they own or rent, say whether other pets already live there, and give a household headcount.
The history comes through three open text fields. "Previous pet ownership experience" and "Why do you want to adopt?" are both required, so no applicant can skate by on checkboxes alone. A third, "If yes, describe your current pets," stays optional and only matters for people who said Yes to having other animals at home. Most applicants finish the whole thing in 10 to 15 minutes.
Reading applications side by side before the meet and greet
