Instead of a single "overall performance" score that managers inflate to avoid conflict, this form breaks evaluation into four distinct 5-point rating scales: Overall Performance, Communication, Technical Skills, and Teamwork. Separating these categories makes it much harder to hand out a blanket "meets expectations" across the board. A developer who writes brilliant code but struggles to communicate with stakeholders will show that gap clearly when Communication and Technical Skills are rated independently.
This separation also gives the employee actionable information. Hearing "you got a 3 out of 5" is useless.
