Eight fields that read like a directory entry
Full name, email, company, and job title are required, so every record is complete enough to publish in an attendee directory without chasing anyone for gaps. Phone and a LinkedIn Profile URL are optional, which keeps the form friendly for people who guard their inbox but are happy to connect online. An Industry dropdown asks each registrant to place themselves in Technology, Finance, Marketing, Healthcare, or Other, and the closing textarea asks "What are you looking to gain?", the one answer that turns a signup into a matchmaking brief.
The fields sit on two content pages between a cover and a thank-you ending, and the template is set up for conversational mode, one question on screen at a time. For a networking event that tone is right: registering already feels like the first friendly exchange of the night.
From a stack of signups to a matched room
Responses collect in Uplup's analytics with per-question breakdowns, so you can see before the event that, say, most of the room is Technology and Finance, and plan introductions accordingly. Export everything to CSV to lay out the printed directory or a seating plan. The open answers about what people want to gain are worth reading the night before; they tell you who is hiring, who is fundraising, and who just moved cities, which is exactly the material a good host uses to open conversations.
If registrations should flow somewhere else, webhooks and the Zapier connection push each new signup to your CRM or event stack as it arrives.
Where the link travels before the doors open
Share the form by direct link or on social, or drop the QR code on printed invites and posters so people can register from their phones at a partner event. It also embeds on your event page, and WordPress sites can place it with the Uplup plugin's shortcode. Add your logo and colors so the form reads as your event from the first screen.
