Every pledge drive produces the same mess: commitments scribbled on cards at an event, amounts promised over the phone, and a spreadsheet three people edit but nobody trusts. When follow-up time comes, half the records are missing a frequency or a program designation, and your team spends a week reconciling instead of thanking donors.
This template fixes the problem at the point of capture. It records everything a follow-up needs in one pass: who is giving, how much, how often, and where the money should go. Donors finish in 3 to 5 minutes, and every submission arrives in the same clean structure.
Nine fields that capture the whole commitment
The basics come first: full name and email are required, phone is optional. Then the form gets to the pledge itself. A required number field records the pledge amount in dollars, and a Frequency dropdown pins down whether the gift is One-time, Monthly, Quarterly, or Annual. A second dropdown, Program to Support, lets donors direct their gift to the General Fund, Education, Healthcare, Emergency Relief, or Other, so designations are settled up front instead of guessed at later.
Two more questions handle details that usually slip through. A required Personal or Corporate choice flags company gifts, with an optional Company Name field to support matching and acknowledgment. A final yes-or-no question, "Would you like a receipt?", tells your team exactly who expects documentation. The 9 fields sit across two content pages, with an ending page to close things out, so the form never feels long.

Each response lands as a complete pledge record. Per-question analytics show how much of your pipeline is monthly versus one-time, and which program is drawing the most support this quarter. Export responses to CSV for your donor database or finance reconciliation, or push new pledges into your CRM automatically through webhooks and Zapier. The receipt question doubles as a task list: filter for everyone who answered Yes and you have your acknowledgment queue ready.
Meeting donors where the ask happens
Share the form as a direct link in appeal messages, print its QR code on pledge cards and event programs, or post it to social channels during a giving day. An embed snippet drops it into your donation page, and if that page runs on WordPress, the Uplup plugin's shortcode does the same job.
