Form Structure

Page 1
Thank You Page

In conversational mode, each field becomes its own page for a focused experience.

Every pledge drive produces the same mess: commitments scribbled on cards at an event, amounts promised over the phone, and a spreadsheet three people edit but nobody trusts. When follow-up time comes, half the records are missing a frequency or a program designation, and your team spends a week reconciling instead of thanking donors.

This template fixes the problem at the point of capture. It records everything a follow-up needs in one pass: who is giving, how much, how often, and where the money should go. Donors finish in 3 to 5 minutes, and every submission arrives in the same clean structure.

Nine fields that capture the whole commitment

The basics come first: full name and email are required, phone is optional. Then the form gets to the pledge itself. A required number field records the pledge amount in dollars, and a Frequency dropdown pins down whether the gift is One-time, Monthly, Quarterly, or Annual. A second dropdown, Program to Support, lets donors direct their gift to the General Fund, Education, Healthcare, Emergency Relief, or Other, so designations are settled up front instead of guessed at later.

Two more questions handle details that usually slip through. A required Personal or Corporate choice flags company gifts, with an optional Company Name field to support matching and acknowledgment. A final yes-or-no question, "Would you like a receipt?", tells your team exactly who expects documentation. The 9 fields sit across two content pages, with an ending page to close things out, so the form never feels long.

Who runs their pledge drives on this

Development directors use it for annual fund appeals, churches for stewardship campaigns, universities for advancement pushes, and community foundations for giving days. Because the Frequency field separates one-time gifts from recurring commitments, the same form serves a single-event ask and a monthly giving program. Small teams like that it's free to use and fully editable: a food bank can swap Healthcare for Food Security in the program list, rename fields, or drop the corporate questions entirely.

From submissions to a follow-up schedule

Each response lands as a complete pledge record. Per-question analytics show how much of your pipeline is monthly versus one-time, and which program is drawing the most support this quarter. Export responses to CSV for your donor database or finance reconciliation, or push new pledges into your CRM automatically through webhooks and Zapier. The receipt question doubles as a task list: filter for everyone who answered Yes and you have your acknowledgment queue ready.

Meeting donors where the ask happens

Share the form as a direct link in appeal messages, print its QR code on pledge cards and event programs, or post it to social channels during a giving day. An embed snippet drops it into your donation page, and if that page runs on WordPress, the Uplup plugin's shortcode does the same job. The template's recommended conversational mode shows one question at a time, which also suits phone pledges: a staff member reads each question aloud and enters answers as the donor responds. Add your logo and colors so the form looks like your organization, not ours.

Who Is This Template For?

This template works for a wide range of goals and industries.

Annual fund pledge drives

Collect commitments for the year in one structured pass. The required amount, Frequency, and Program to Support fields mean every pledge arrives with the designation and schedule your finance team needs, and CSV export moves the whole drive into your donor database at once.

Monthly giving program signups

The Frequency dropdown's Monthly, Quarterly, and Annual options make this a natural intake form for recurring giving. Per-question analytics show how many donors chose an ongoing commitment versus a one-time gift, so you can measure how the program is growing.

Giving days and event pledges

Print the form's QR code on pledge cards and programs, or share the link on social channels while the event runs. Staff taking pledges in person or by phone can use conversational mode to walk a donor through the questions one at a time.

Key Features

Four pledge frequencies built in

A required Frequency dropdown records whether the gift is One-time, Monthly, Quarterly, or Annual, so recurring commitments are separated from single gifts the moment they're made.

Program designation up front

Donors direct their pledge to the General Fund, Education, Healthcare, Emergency Relief, or Other before submitting. Rename or replace the options to match your own program list.

Corporate gift flagging

A required Personal or Corporate question, paired with an optional Company Name field, identifies business donations for matching programs and proper acknowledgment.

Receipt preference on record

The closing "Would you like a receipt?" question captures who expects documentation, giving your team a ready-made acknowledgment list instead of a guessing game.

Frequently Asked Questions

Is the Donation Pledge Form free to use?
Yes. It runs on Uplup's free plan, so you can edit the fields, collect pledge responses, and export them to CSV without entering a card.
Can I change the programs donors choose from?
Yes. The Program to Support dropdown ships with General Fund, Education, Healthcare, Emergency Relief, and Other, and every option is editable. Rename them to match your organization's programs, add new ones, or trim the list down.
Does the form handle corporate pledges?
Yes. A required question asks whether the gift is Personal or Corporate, and an optional Company Name field records the business behind corporate gifts, which helps with matching programs and public recognition.
How long does the pledge form take to complete?
About 3 to 5 minutes. There are 9 fields over two content pages, and only the essentials are required: name, email, pledge amount, frequency, program, donation type, and receipt preference. Phone number and company name are optional.
