A week before the event you're reconciling it all against a catering deadline, and the numbers never quite agree.

The Charity Gala Registration template pulls that sprawl into one place. Guests choose a ticket, flag dietary requirements, and add special requests in a single pass, and every answer lands in the same response table. Eight fields, from a single seat to a table of ten

The form starts with the essentials. Full name and email are required, while phone and Company/Organization stay optional, so corporate guests can tie their registration to a sponsor without forcing that step on individuals. Then comes the field doing the heavy lifting: a required Ticket Type selector with four options, Individual, Couple, Table of 8, and Table of 10. Selling whole tables through the same form as single seats means you keep one guest list instead of two.

A required Dietary Requirements question offers None, Vegetarian, Vegan, Gluten-free, and Other, which turns your catering count into a per-question breakdown you can read in the analytics or hand to the venue as a CSV export. Next, a sponsorship interest question includes a useful third answer, Tell me more, which catches warm prospects who aren't ready to say yes on the spot. An open Special Requests box rounds out the eight fields. In the recommended traditional mode, guests move page by page from a cover through the detail pages to a payment step and a closing confirmation, one decision at a time.

Shape it to your event before the invitations print

Everything here is editable. Rename the ticket tiers to match your price points, swap Table of 8 for whatever your venue seats, or add an auction paddle field if bidding is part of the night. Custom branding puts your organization's logo and colors on every page, which matters when donors are about to enter payment details.

When it's ready, share a direct link in your email campaign, print a QR code on the physical invitations, embed the form on your website, or drop it into WordPress with the Uplup plugin's shortcode. Webhooks and Zapier can push each registration to your donor CRM as it arrives.
