Form Builder Interface Overview
The Uplup Form Builder is organized around five top tabs – Create, Logic, Connect, Share, and Results – with the building experience itself laid out across three main areas.

1. Field Library (Left Sidebar)
The left sidebar contains an Add Page button at the top, followed by accordion sections for every field group, plus inline Design and Security panels. Drag any field onto the canvas to add it, or click it to insert at the cursor.
- Text – Short Answer, Long Answer, Rich Text, Number, Website
- Contact – Email, Phone, Full Name, Address, Birthday, Date & Time, Time, Date Range, Country, Company Information, Social Media Links
- Selection – Multiple Choice, Checkbox, Dropdown, Multi-Select, Picture Choice, Yes/No, Terms & Conditions
- Feedback – Star Rating, NPS Score, Likert Scale, Slider, Matrix, Ranking
- Uploads – File Upload, Signature
- Content – Heading, Paragraph, Image, Video, Icon, Divider, Spacer
The Add Page dropdown lets you insert a Form Page, Cover Page, Finish Page, or Payment Page (Pro+).
2. Form Canvas (Center)
The center area shows a live preview of your form. Drag fields from the left sidebar to arrange them, or click any field to select it. Selected fields show an inline logic popover for field-level conditional rules.
3. Field Settings (Right Panel)
When you select a field, the right panel shows configuration options for that field type – including label, placeholder, help text, required toggle, validation rules, and field-specific options (such as the choice list for a Dropdown).
Top Tabs
- Create – Build the form (this view)
- Logic – Manage conditional logic in Rules or Flowchart view
- Connect – Wire up CRMs, analytics, Stripe, Zapier, n8n, MCP, webhooks, and email notifications
- Share – Get the public URL, embed code, QR code, and social share buttons
- Results – View submissions, analytics, and (if a Payment Page is enabled) revenue
Form Types: Classic vs Conversational
When you start a new form, Uplup asks how your fields should be displayed. You can change this later from the Design panel.

Classic Mode
Traditional layout with all fields visible together on a scrollable page. Best for:
- Contact forms
- Application forms
- Registration forms
- Order forms
Conversational Mode
One field at a time, chat-like experience. Best for:
- Surveys
- Lead generation
- Feedback forms
- Long onboarding flows

Quick Start: Create a Form in 5 Minutes
Here’s a quick overview of the form creation process. For detailed instructions, see the Getting Started guide.
Step 1: Open the Create Menu
From your dashboard, click the Create button in the top-left and choose New Form or Survey.
Step 2: Choose a Starting Point
You can start from scratch, generate a form with AI, or pick from the template library. Templates include pre-built fields and styling that you can customize.
Step 3: Pick a Layout
Choose One at a time for a Conversational form or All on one page for a Classic form.

Step 4: Add and Configure Fields
On the Create tab, drag fields from the left sidebar onto the canvas. Click any field to configure its label, placeholder, validation, and other settings in the right panel.
Step 5: Style and Secure Your Form
Open the Design accordion in the left sidebar to choose a theme, customize colors and fonts, set the layout, and upload a background or logo. Open the Security accordion to configure submission limits, spam protection, and country or U.S. state restrictions (Business plan).
Step 6: Connect Integrations
Switch to the Connect tab to wire up CRMs, analytics, Stripe payments, Zapier, n8n, webhooks, and the respondent confirmation / admin notification emails.

Step 7: Share Your Form
Open the Share tab to copy your public link, grab the embed snippet, download a QR code, or share to Facebook, X, LinkedIn, WhatsApp, or email.
