In return you get a consistent record for every company that wants to sell to you: who they are, what they offer, and what it takes to order from them. Compare that with the usual alternative, a pile of unstructured intro emails with PDFs attached, and the case for a structured application makes itself. The form runs as a cover page, two content pages, and an ending page, so applicants always know where they are.
From contact details to minimum order quantity, one record per supplier
The form opens with the basics: full name, email, and company name are required, while phone number and company website are optional. A Product Category dropdown sorts applicants into Electronics, Clothing, Food, Home Goods, or Other, so you can filter the pipeline by what you actually buy. A required Products/Services Offered text area gives each supplier room to describe their range in their own words, and an optional Minimum Order Quantity field surfaces the dealbreaker number early, before anyone books a call. There's also an optional Product Catalog field; catalogs come through attached as links, so your team can open them straight from the response.
