Those are the six options behind the Reason dropdown in this template, and that one dropdown does more work than any other part of a returns process. When a customer has to pick a reason instead of typing a paragraph, you stop decoding rambling emails and start seeing patterns: which products come back, why, and what the buyer wants instead.
The template is a ready-made return and exchange request form with 9 fields across two content pages, followed by an ending page that confirms the submission. Nine fields, six reason codes, three ways to make it right
Six of the nine fields are required: name, email, Order Number, Product Name, the Reason dropdown, and a Preferred Resolution choice. That last one is a radio pick between Refund, Exchange, and Store Credit, so every submission arrives with the customer's preferred outcome already stated. You never have to guess whether they want their money back or a replacement.
The other three fields are optional on purpose. Purchase Date helps you check the return window without making customers dig out a receipt before they can submit. Additional Details leaves room for context that does not fit a dropdown. And the Photo of Product field lets a shopper back up a defect claim with an image, which comes through with the response as a link you open while reviewing the case.
