An employee who fills this out spends a few minutes vouching for someone they trust, and maybe earns a referral bonus for it. You get something better than a name forwarded in a chat thread: candidate details, relationship context, and a written case for the role, all in one submission. That trade is the whole point of this template, and every one of its eight fields serves one side of it or the other.

Eight fields that capture both sides of the referral

The form opens with the referrer: full name and Work Email, both required, so bonus payouts and follow-up questions always land with the right person. Then it turns to the candidate: name, Candidate Email, and an optional Candidate Phone, followed by Position Referred For so recruiters know exactly which opening to screen against.

Two fields do the real filtering. A required Relationship dropdown asks how the referrer knows the candidate, with options ranging from Former Colleague to Family. And a required long-answer question, Why would they be a good fit?, means nobody can submit a bare name. Frequently Asked Questions

Do employees have to provide the candidate's phone number?
No. Candidate Phone is the one optional field among the eight. Candidate Name, Candidate Email, and Position Referred For are all required, so recruiters always have a way to reach the person and know which role they are being considered for, even without a number.
How does my recruiting team get the referrals out of Uplup?
