The form opens with the referrer identification: name and work email. This matters because the referral needs to be traceable. The next block captures candidate information: name, email, phone (optional), and the position they are being referred for. Position Referred For is a free-text field rather than a dropdown, which is a deliberate choice. Neither is disqualifying, but the context helps prioritize outreach.

The final required field asks "Why would they be a good fit?" as an open textarea. This is the most valuable field on the form. A referral that says "worked with them for 3 years at Acme, they led the migration to AWS and trained the entire backend team" gives the recruiter a concrete reason to prioritize that candidate. A referral that says "they are great" does not.
Encouraging employees to write two or three sentences about the candidate also filters out low-effort referrals. Programs that only ask for a name and resume get flooded with submissions from people chasing the referral bonus without real conviction. Adding a "why" field raises the quality of every submission without discouraging genuine referrals.
