The form collects nine fields across three content pages, and most employees finish it in 5 to 8 minutes. It starts with the basics: full name, work email, and the name of the approving manager. A Department dropdown covers Engineering, Marketing, Sales, HR, Finance, and Operations, so responses sort cleanly when you review them later.
Then comes the substance. The Request Type field pins down the arrangement, an optional Start Date and an End Date field marked "if temporary" frame the timeline, and a required Justification box asks the employee to make their case in their own words. A final optional field asks about the home office setup, useful when equipment or connectivity affects the decision. The template runs well in conversational mode, presenting one question at a time so the request feels like a guided conversation rather than paperwork.
