Form Structure
In conversational mode, each field becomes its own page for a focused experience.
"How do you plan to promote our products?" is the question most affiliate managers wish they'd asked before handing out tracking links. This template asks it up front, alongside audience numbers and content samples, so you can spot real promoters before commission ever changes hands.
Nine answers that separate promoters from link collectors
The form opens with a cover page, then spreads nine fields across two content pages so applicants aren't staring at one long wall of inputs. The first stretch collects the basics: full name, email, and a required Website or Blog URL. A checkbox list follows, asking which platforms the applicant is active on, with Instagram, YouTube, TikTok, Twitter / X, and Blog / Newsletter as the options.
From there the questions dig into fit. Applicants enter their Total Audience Size as a number, pick a Primary Content Niche from a dropdown that covers Technology, Business, Lifestyle, Health & Fitness, Finance, and Other, then write out two longer answers: a description of their audience demographics and their plan for promoting your products. A Sample Content Link closes the form, and it's the only field that isn't required, so creators without a public portfolio can still apply.
Uplup recommends running this one in conversational mode, which presents one question at a time. Applicants take 8 to 12 minutes to finish, which is long enough to filter out drive-by signups without scaring off serious partners.
Who gets the most out of this application
Ecommerce brands use it to vet influencers before a product launch. SaaS and subscription companies use it to build referral partner benches, where the audience demographics answer matters more than raw follower totals. Agencies managing affiliate programs for clients use it as a standard intake so every applicant is judged on the same nine data points.
The pattern is the same in each case: reach, niche, and intent captured in one place, so approval decisions take minutes instead of email threads.
Reshape it around your own program
Every field is editable. Swap the niche options for categories that match your catalog, add a field for promo code preferences, or make the sample link required if you only want established creators. You can also restyle the form with your logo and brand colors before sharing it by link, QR code, or an embed on your affiliates page. WordPress users can drop it in with a shortcode via the Uplup plugin.
Responses land in a dashboard with per-question breakdowns, and you can export everything to CSV or push each submission to your CRM through webhooks or Zapier. The template ships on Uplup's Pro plan, and once it's live you can keep tuning questions without breaking the share link.
Who Is This Template For?
This template works for a wide range of goals and industries.
Influencer vetting before a launch
Ecommerce brands collect audience size, platform mix, and a written promotion plan from every creator, so launch partners are chosen on evidence instead of follower counts alone.
SaaS referral partner recruiting
Subscription companies lean on the audience demographics and Primary Content Niche answers to find partners whose readers actually match their buyer, not just the biggest accounts.
Agency-run affiliate intake
Agencies managing programs for clients use the form as a standard application, so every applicant across every client is scored on the same nine data points.
Key Features
Reach and intent in one submission
Total Audience Size, a platform checklist from Instagram to Blog / Newsletter, a niche dropdown, and two written answers give you numbers and reasoning side by side.
