How the Feedback Form is Structured
The questions move from broad to specific in a deliberate order. Question one asks for an overall experience rating on a four-point scale, from "Exceeded expectations" down to "Below expectations." Question two measures referral likelihood, which functions as a built-in NPS indicator. These two scored questions give you quantifiable data you can benchmark.
Questions three and four use checkbox-style multi-select fields. One asks what the respondent liked most (product quality, customer support, ease of use, value, speed), and the other asks what could improve (quality, support response time, usability, pricing, communication). You can adjust these point values to weight certain questions more heavily. If referral likelihood matters more to your team than overall satisfaction, increase the points on that question. The scoring gives you a single number per response that is easy to track in a spreadsheet, dashboard, or CRM.
