Form Structure
Multi-page layout keeps your form organized and easy to complete.
Reviewing 500 startup applications when half of them are missing basic information is not a selection process. It is an excavation project. Accelerator teams spend more time chasing incomplete submissions than evaluating promising companies. A structured application form fixes this by requiring every founder to provide the same core information in the same format, so your review committee can compare startups fairly and move faster.
This template has 35 fields across 6 pages in traditional mode, designed for thorough completion rather than quick submission. It captures company details, founder contact information, sector and stage of development, an elevator pitch, the problem being solved, team background, revenue data, accelerator goals, and a required pitch deck upload.
What the Application Covers and Why Each Section Matters
The form starts with company name, founder name, email, and optional website. This baseline identifies the applicant and lets your team verify the company exists. The sector field offers six categories (SaaS, FinTech, HealthTech, E-Commerce, AI/ML, Other) that align with how most accelerators organize their cohorts and mentorship tracks.
The stage of development selector (Idea, MVP, Early Revenue, Growth Stage) is one of the most important fields in the application. It immediately tells your team whether this startup fits your program's target stage. An accelerator focused on post-MVP companies can filter out idea-stage applicants before reading a single pitch.
The elevator pitch field asks for two to three sentences describing what the startup does. This is the field your reviewers will read first when scanning applications, so the character constraint forces founders to be concise. The longer problem description field follows, asking founders to articulate the specific pain point and target customer. Together, these two responses give your committee the "what" and the "why" without requiring a full business plan.
Team Profiles, Traction, and the Pitch Deck
Team background is where applications get interesting. The open-text field asks for co-founder bios and key team members, which lets your reviewers assess whether this team has the experience and complementary skills to execute. Team size is captured as a number field for quick filtering.
Monthly revenue is optional but revealing. A startup generating $10,000 per month tells a different story than one at zero, and both can be appropriate depending on your program. Making the field optional means pre-revenue startups are not discouraged from applying.
The final required field is a pitch deck upload. Requiring a PDF deck ensures your committee has a visual summary of the business to pair with the written responses. The accelerator goals field asks founders what they hope to gain, which helps your team match applicants to the right mentors and resources.
Programs That Use This Application Format
Y Combinator-style accelerators use forms like this to manage batch applications with hundreds of submissions per cycle. University incubators embed it on their program page for student and alumni founders. Corporate innovation labs use it to source startups for pilot programs and partnership opportunities. Regional economic development organizations use it for grant-backed accelerator programs where the application also serves as a compliance document.

Key Features
Sector and Stage Filtering
Dropdown fields for industry sector and development stage let your team instantly filter applicants by fit. Sort by SaaS companies at the MVP stage or HealthTech startups with early revenue without reading every application.
Required Pitch Deck Upload
A file upload field requires applicants to attach their pitch deck as a PDF. Your committee gets a visual summary alongside written responses, which speeds up the initial review process.
Elevator Pitch and Problem Statement Fields
Two separate text areas capture the concise what-we-do pitch and the deeper problem-market explanation. Reviewers can scan elevator pitches quickly and dive into problem statements for shortlisted companies.
Monthly Revenue Data Point
An optional number field captures current monthly revenue. This single data point adds crucial context to every application without burdening pre-revenue startups with detailed financial reporting.
Six-Page Traditional Layout for Thorough Responses
The multi-page traditional format organizes the application into logical sections. Founders complete it like a structured proposal rather than a quick form, which produces more thoughtful responses.
