Form Structure

A parent tapping through this on their phone gives up three to four minutes and leaves knowing exactly what they offered: a specific event, a morning or afternoon slot, and a note about what they're good at. The coordinator on the other end gets something far more useful than a thread of well-meaning replies: a structured record showing who can cover the Book Fair, who is free all day, and whose background check is already on file.
From replies to a duty roster you can actually publish
Every submission lands already sorted. The Event/Activity question has each volunteer pick from Field Day, Book Fair, Science Fair, Fundraiser, Classroom Helper, or Other, and the Time Available question pins them to Morning, Afternoon, or Full Day. Open the per-question breakdowns and you can see at a glance which events are covered and which are running thin, then aim your recruiting at the gaps.
When it's time to build the schedule, export everything to CSV and sort by event and time slot. If your school coordinates elsewhere, webhooks and a Zapier connection can pass each new signup along the moment it arrives.
Nine fields, each with a job
The required entries are the ones a coordinator cannot work without: full name, email, a Parent/Guardian Name, the event, the time window, and a Yes or No answer to Background Check on File. That last one matters more than it looks. Knowing in advance who still needs screening saves an awkward scramble the week before the event.
The rest is optional by design: a phone number, specific Available Date(s), and a Skills/Experience box where someone can mention they ran the cash box last year or own a popcorn machine. Volunteers in a hurry skip past them. Volunteers with something extra to offer tell you about it.
The whole thing sits on a single content page with an ending page to close things out, and it's built to run in conversational mode, one question at a time, which suits a parent filling it in from the pickup line. Expect three to four minutes end to end.
Rename Field Day to whatever your calendar says
Every option list is editable. If your school runs a Fall Carnival instead of a Field Day, change the label and move on. Add events, split Classroom Helper into reading groups and art days, make the phone number required, or drop the date field entirely for a single-day event. Your colors and logo replace the defaults, so the form reads as the school's, not as a generic tool.
Distribution follows however your community already communicates: a link in the weekly newsletter, a QR code on the flyer that goes home in backpacks, an embed on the school website, or a shortcode from the Uplup WordPress plugin when the PTA site runs on WordPress.
