Form Structure
In conversational mode, each field becomes its own page for a focused experience.
Someone wants to see your product, so they email sales. Sales replies with three follow-up questions, the prospect answers one, and by the time a time slot goes out they have already watched a competitor's demo. Most demo pipelines leak in exactly that spot, the gap between "I'm interested" and a call your team can actually prepare for.
This template closes the gap. It is a two-page form, the request itself plus an ending page, that collects seven answers in an estimated 2 to 4 minutes and hands sales a request worth prioritizing: who the person is, how big their company is, and what they are trying to get done.
From vague interest to a call your AE can prep for
Say you sell software to mid-market teams. A prospect hits your pricing page, clicks through to this form, and submits. Before anyone books anything, you know they are a marketing manager at a company with 51-200 employees, their primary use case is lead generation, and they have written two sentences about what they hope to achieve. Your account executive opens the call with a plan instead of running a discovery session disguised as a demo.
That context also tells you who not to chase. A student side project and a 1000+ employee rollout look identical in a bare contact form. Here they sort themselves out before you spend a calendar slot on either.
Seven fields, and each one earns its place
Four required text and email fields anchor the request to a real person at a real business: full name, work email address, company name, and job title. Two required dropdowns then do the qualifying. Company Size offers five ranges, from 1-10 employees up to 1000+, and Primary Use Case lets prospects pick from options like Lead Generation, Customer Feedback, and Event Registration, or choose Other.
The one optional field is an open textarea, "What are you hoping to achieve?". Nobody abandons the form over it, and the prospects who answer usually tell you exactly how to sell to them.
The template's recommended conversational mode presents one question at a time, which keeps a seven-field ask feeling short. A progress bar and back navigation are there if you want them.
Reshape it around your own sales motion
Every field is editable, so swap the Primary Use Case options for the jobs your product actually does, add a budget question, or drop job title if it never changes your routing. Apply your logo and brand colors, then share the form by direct link or QR code, embed it on a landing page, or add it to a WordPress site with the Uplup plugin's shortcode.
Submissions collect in your dashboard with per-question breakdowns, and you can export them to CSV or push each new request into your CRM through webhooks or Zapier. The template sits on Uplup's free plan, so you can publish it today and start filling a qualified demo queue.
Who Is This Template For?
This template works for a wide range of goals and industries.
B2B SaaS sales teams
Swap the generic contact form on your pricing page for a request that arrives pre-sorted by company size and use case, so SDRs route enterprise interest and self-serve prospects differently from the first touch.
Founder-led sales at early startups
When the founder runs every demo personally, the optional "What are you hoping to achieve?" answer and the company details mean each call starts from context instead of twenty minutes of discovery.
Demand gen campaigns with a demo CTA
Embed the form on campaign landing pages, then use per-question breakdowns to see which channels bring 201-1000 employee prospects and which mostly deliver tire kickers.
Key Features
Qualification built into the fields
Required Company Size and Primary Use Case dropdowns sort every request before a human reads it, alongside work email, company name, and job title to tie the demo to a real buyer.
Conversational mode that keeps it short
The recommended one-question-at-a-time flow makes seven fields feel like a 2 to 4 minute chat, with a progress bar and back navigation available if respondents need them.
Requests routed where sales already works
Submissions land in your dashboard with per-question breakdowns, export to CSV in one click, or flow straight into your CRM through webhooks and Zapier.
