Three fields identify the client. Full name and email are required, phone is optional, so people who would rather not take calls can still book. Then a Service Type dropdown presents four choices: Consultation, Full Service, Quick Session, or Follow-up.

Scheduling is handled with a light touch. There is an optional Preferred Date picker and a required Preferred Time selection with three windows: Morning, Afternoon, or Evening. Asking for a window instead of an exact slot means you confirm the final time yourself, which avoids double bookings without needing a live calendar. The whole form sits on a single content page followed by an ending page, and it is set up for conversational mode, so questions appear one at a time, closer to a chat than to paperwork.
