Licensing boards, permit offices, trade associations, and certification bodies all end up drafting the same document: an application that confirms who someone is, what they trained in, and whether anything in their history needs a second look. This template is that document already built, a 12 field online application that takes 15 to 20 minutes to complete and works for new licenses, renewals, and reinstatements alike.

Built for the desks that issue credentials

The License Type menu covers Professional, Business, Contractor, and Trade Licenses, which means a state board, a county clerk, and a guild certifying skilled trades can all run intake from the same form. The Application Type question then splits New Applications from Renewals and Reinstatements, so returning licensees aren't pushed through a process designed for first-timers, and your team can filter each stream separately in the responses.

It also fits organizations that license internally: a franchise network credentialing new operators, or a professional body authorizing members to carry its certification mark.

Identity up front, disclosures at the end

The application runs across four pages in a deliberate order. It opens with the basics: full legal name, email, phone, and date of birth, the details you'll check against official records. The middle of the form covers qualifications, with a free text field for education and training credentials and a number field for years of professional experience. Two document fields then ask applicants to attach their government-issued ID and proof of education, which arrive with the submission as links to the files.

The last substantive question is the one licensing teams care about most: "Have you ever had a license revoked?" It sits beside an explanation field for anyone who answers yes, and that explanation box is the only one of the twelve fields that isn't required. Everything else must be answered before the form will submit.

From your licensing page to a finished record

Publish the form and share it as a direct link, print its QR code on renewal notices, or embed it on your licensing page. If your site runs on WordPress, the Uplup plugin adds a shortcode for it. Because the template's recommended mode is traditional pages rather than one question at a time, applicants see a progress bar and can step back to an earlier page to correct an answer before submitting.

On the back end, every submission lands in response analytics with per-question breakdowns, exports to CSV for your records, and can push into case management software through webhooks or Zapier. Publishing requires a Business plan workspace, and once the template is in yours, every field, option, and page is yours to edit.
