The first section collects name, email, and date of birth. Then it moves into the academic core: School Name, Major or Field of Study, and Current GPA as a required numeric field. Having GPA as a number input (rather than a text field) means committees can sort and filter applicants by GPA directly in the dashboard without cleaning up data first.
Annual Household Income uses a dropdown with five ranges, from Under $25,000 to Over $100,000. This field is sensitive, but presenting it as a range rather than an exact dollar amount reduces friction and still gives committees enough information to assess financial need. Organizations that offer need-based scholarships can filter applications by income bracket before reading essays, which makes the initial screening pass significantly faster.
Essays, Transcripts, and Letters That Stay Attached
The essay prompt asks "How will this scholarship help you?" as a required textarea.
