Applicants move through a cover page, three content pages, and an ending page, and the whole thing takes an estimated 10 to 15 minutes to complete.

Nine fields that mirror an underwriting checklist

The form opens the file with the basics: full name and email are required, phone number is optional. From there it gets into the numbers. Loan Amount ($) and Annual Income ($) are both required number fields, so applicants hand you clean figures you can compare across files instead of prose you have to interpret.

Three dropdowns do the profiling work. Loan Purpose covers Personal, Business, Auto, Home, Education, and Other. Employment Status offers Employed, Self-employed, Unemployed, and Retired, paired with an optional Employer text field for the specifics.
